"The table gives a clear visual indication of the chance of a woman surviving for 10 years if she does - or doesn't - have her routine Pap smears. It is apparent that to an individual woman the absolute benefit of having a Pap smear is small.
Table 1 - to show the benefit gained by women attending the NHS cervical cancer-screening programme ________________________________________________________ Age No of Alive 10 Alive 10 at start women alive years later years later of 10 at start of if they attend if they do year period 10 year NHSCSP not attend period NHSCSP _______________________________________________________ 25 10,000 9963 9962 35 10,000 9863 9859 45 10,000 9713 9708 55 10,000 9457 9450 ______________________________________________________
Women must also be told about the inaccuracy of the Pap test and the disadvantages of being tested. For instance, they must be told that it is quite likely that the laboratories that examine her smear will quite likely miss abnormalities if they are present. We suggest that a statement suggesting that "even in the best laboratories, at least 5-15% of abnormal smears may be reported as normal", is appropriate. They should also be told that there is about a 7% chance that they will require further testing and about a 3% chance that they will need to attend a hospital and undergo colposcopy."
In the NHS cervical screening programme around 1000 women need to be screened for 35 years to prevent one death Over 80% of women with high grade cervical intraepithelial neoplasia will not develop invasive cancer, but all need to be treated "
" I feel compelled to say that I doubt that 95% of American women have had a cervical biopsy or colposcopy. I haven't seen actual statistics for that. If anyone has documentation for the frequency of cervical biopsies please post it. October 20, 2009 8:50 PM Steve said...
I've read that also...anything from 77% to 95%. It's a high figure whether you take the lower or higher estimate. The more often you screen, the higher the risk of colposcopy. US women have a lot more pap smears than women from other countries and this is reflected in your colposcopy rates. The R. De May article in the American Journal of Clinical Pathology was attached to a post some time ago. (or, did I see it elsewhere?) I bookmarked it anyway and have included the link for your reference. He mentions high colposcopy rates. I know the 77%-78% figure is right for Aussie women. I've seen it in a few papers. That figure has never been released to women and isn't generally known. I've never seen it mentioned in the media or in the Papscreen leaflets. Laura Koutsky published a paper mentioning the 77% figure. This figure seems to have been accepted by the profession and is often cited in other research papers on the subject. I think it's a figure that should be released to women.
Laura Koutsky's article was published in 2004 and this research includes the 77% figure. She says the US figure is "at least as high". The DeMay article puts it higher than that and so does a few other research papers I've read over the years. I'll post them, if I can find them. Our rates are the end result of biannual screening from a year or two after the initiation of sexual activity. (usually around 18-20 years) Hope this helps. RUTIN NŐGYÓGYÁSZATI VIZSGÁLATOK ÉS FOGAMZÁSGÁTLÓ TABLETTA:
Although hormonal contraceptives arenotrecommended for women with some serious medical conditions, the problems that make their use unwise are effectively identified through medical history and a simple blood pressure measurement. Hormonal contraceptives can safely be started based on medical history review and a blood pressure check. For most women no further evaluation is needed before making a decision to use them, said George F. Sawaya, MD, UCSF assistant professor of obstetrics, gynecology and reproductive sciences and research coordinator for the UCSF-Stanford Evidence-based Practice Center.
Clinical Breast and Pelvic Examination Requirements for Hormonal Contraception: Current Practice vs Evidence Felicia H. Stewart; Cynthia C. Harper; Charlotte E. Ellertson; David A. Grimes; George F. Sawaya; James Trussell JAMA. 2001;285:2232-2239.
The summary is: Clinical breast and pelvic examinations are commonly accepted practices prior to provision of hormonal contraception. Such examinations, however, may reduce access to highly effective contraceptive methods, and may therefore increase womens overall health risks. These unnecessary requirements also involve ethical considerations and unwittingly reinforce the widely held but incorrect perception that hormonal contraceptive methods are dangerous. This article reviews and summarizes the relevant medical literature and policy statements from major organizations active in the field of contraception. Consensus developed during the last decade supports a change in practice: hormonal contraception can safely be provided based on careful review of medical history and blood pressure measurement. For most women, no further evaluation is necessary.

"J. Marion Sims, born James Marion Sims (January 25, 1813 November 13, 1883) was a surgical pioneer, considered the father of American gynecology.[1] Modern historians argue about his legacy as Sims used slaves as experimental subjects.[2]...Sims treated three Alabamian slave women - Anarcha, Betsy, and Lucy - all of whom may have been suffering from fistula problems, to develop new techniques to repair this condition.[2] From 1845 to 1849 he experimented on them, operating on one of them 30 times (it remains unclear if this was necessary due to stitching failure, or if Sims did it deliberately). Although anesthesia had recently become available it was rarely used as yet; whatever the reason, we know he did not use it on the slave women.[1][2] After the extensive experiments and difficulties, Sims finally perfected his technique and repaired the fistulas successfully in Anarcha. He then repaired several other slave women.[3]. It was only after the success of the early experiments on the slaves that Sims attempted the procedure on Caucasian women with fistulas, this time with anesthesia."
Sztk-ba bemehetek terhessegi vizsgalat celjabol?
Kicsit egocentrikusnak tűnő a hozzáállásod.
Dobod? A gyerekedet is?
Egyszerűen másként hat rám a dolog.
Most függesszük fel a témát, holnap egész nap dolgozom.
Egyszerűen másként hat rám a dolog.
Elmondta, rákerestem a neten; innen ered. (Amúgy nagy nehezen veszi rá magát a dologra)
